编者按:患者到医院就诊后,医生最关心的是患者离开医院后是否遵循了医嘱配合治疗,因为这将大大影响诊疗效果。但却苦于院中与院后信息的不通畅,只等依靠复诊时简单的问询得知。近日,来自美国的healthfinch公司发布最新的工作流程自动化应用软件:Chirp™。Chirp™是一款完全自动化的患者沟通工具,它能积极主动地通知患者何时要前来就诊或者上化验室做一些化验,从而帮助患者遵守医疗方案。

一些医疗机构和诊所正在努力识别哪些患者可能没有在遵守医疗方案,它们现在可以求助一款新的应用软件。healthfinch这家公司曾开发了屡获奖项的处方补充请求应用软件(Swoop™),近日发布最新的工作流程自动化应用软件:Chirp™

Chirp™是一款完全自动化的患者沟通工具,它能积极主动地通知患者何时要前来就诊或者上化验室做一些化验,从而帮助患者遵守医疗方案。不同于其他的患者沟通工具,Chirp使用Swoop中内置的用药方案,不管患者有没有患有慢性疾病,都能够发现治疗中的每个缺口。目前,大多数人口健康计划只能跟踪严重疾病状态,关注患严重疾病的患者。Swoop结合Chirp,可通过信函、安全文本或电话来触发安排通知,甚至可用于已痊愈的患者。

healthfinch首席执行官兼联合创始人Jonathan Baran解释:“彻底改变医疗服务的关键在于,对患者要积极主动,同时简化医生和护士的工作流程。Swoop可以发现约25%的医疗缺口——否则因为人工流程而疏忽的这些医疗缺口,容易使患者需要再次接受复查。”

healthfinch有一个客户是一家拥有300余名初级保健医疗人员的大型医疗机构,它发现Swoop每周可识别数百名需要通知的患者,以便让他们遵守治疗方案。healthfinch首席运营官Sanaz Cordes医学博士解释:“现在,Chirp可以视情况自动采取行动,通知患者他们需要安排再检查,而不是使用宝贵的人力来打印和装封信函,然后邮寄给这些病人,或者逐一打电话过去。这不仅为诊所带来了亟需的效率提升,还是一种非常出色的风险缓解工具。”

Healthcare systems and medical practices that are struggling to identify patients who may not be adhering to their care plans have a new application to turn to for help. healthfinch, the company behind the award-winning application for prescription refill requests (Swoop™), announces the release of its newest workflow automation application—Chirp™.

Chirp™ is a fully automated, patient communication tool that drives adherence to care plans by proactively notifying patients when they must come in for an office visit or stop by the lab for some tests. Unlike other patient communication tools, Chirp uses the medication protocols embedded in Swoop to catch every gap in care, regardless of whether or not a patient has a chronic condition. Currently, most population health initiatives track only major disease states and work to keep those patients on track. Swoop, combined with Chirp, triggers scheduling notifications via letter, secure text or phone calls, even for healthy patients.

healthfinch CEO and co-founder Jonathan Baran explains, “The key to transforming care delivery is getting proactive with patients while, at the same time, simplifying physician and nurse workflows. Swoop catches about 25% of care gaps that may otherwise be missed with a manual process, hence the need to get those patients in for rechecks.” 

One healthfinch client, a major health system with over 300 primary care providers, finds that Swoop identifies hundreds of patients each week that require notification to keep them adherent to their care plans. “Now, instead of using valuable manpower to print, stuff and send letters to these patients or making calls, Chirp automatically springs into action to notify the patient that they need to schedule a recheck. This is not only a much needed efficiency gain for practices, but it’s an incredible risk mitigation tool,” explains healthfinch COO, Sanaz Cordes, MD.

Chirp is available as an enhancement to Swoop and is the first in a series of several new care redesign apps slated for release in 2015.
