纵观HL7标准版本发展,David.Hay将亮相6月CHINC大会系统阐释“消息”标准

The recent announcement that HL7 is to make its standards ‘no cost to use’ is a good time to reflect on the impact that HL7 standards have had over the past 25 years, and some thoughts on the future.

The first HL7 standards that were widely available were the ‘version 2’ standards that were primarily intended for messaging use within a hospital – often for administrative purposes – the so-called ‘ADT’ (Admission Discharge Transfer) messages. Version 2 (or v2 as they came to be known) were extremely successful as they were designed by those who were actually involved in building these systems, and offered both standardized messages (exactly where to put patient identity for example) and the ability to customize the messages for purposes that were specific to a particular implementation.

It was this latter point – the ability to customize messages using segments that had a prefix of ‘Z’ (and so were inevitable called Z-segments) that led to the major issues with V2 – different people represented the same information in different ways and so it was necessary for implementers to discuss in advance how they intended to use the standard – and what information they would put in what ‘slots’ in the message. This led to the saying ‘When you’ve seen one v2 implementation, you’ve seen just one v2 implementation’.

To resolve this issue, in the early 2000s HL7 started on a new version that was based on a commonly agreed Information Model – the Reference Information Model (RIM) – upon which all v3 artefacts were to be based. By using the same model, both sender and recipient should be able to more easily understand the message making regional semantic interoperability much easier than if there were no common model.

However, with the exception of CDA (the Clinical Document Architecture), version 3 has not been widely adopted. There are many reasons for this – the messaging formats developed to support the RIM is complex and difficult to develop against, design of v3 messages is complex, tooling is only now becoming available but most importantly there was little incentive for existing v2 users – particularly within institutions – to change, which is always a costly exercise.

CDA however, (and its derivatives CCD and C-CDA) have been very successful, largely because it more clearly defined the content, and it also defined a standard XML schema that all CDA documents would adhere to, and had tooling to help them. Ironically, CCD was inspired by a separate standard (CCR) which defined the information needed when patients were transitioning between care providers, though is still based on the RIM.

Although extremely successful, CDA is not without it’s issues. It’s XML structure is complex and can be difficult to extend and implement. This applies particularly for mobile devices that are an increasingly important component in healthcare. It is a document rather than messaging paradigm, and so does not easily meet the needs of real-time interaction or many existing v2 infrastructures without additional components such as web services. Indeed, in many deployments V2 is used as a carrier for CDA. In addition, a CDA message is a very basic structure, and requires significant definition and agreement of components to be truly re-usable.

FHIR (Fast Health Interoperability Resources) is the new kid on the block, having been around for only a year, but is firmly designed with the implementer in mind. It uses the concept of simple ‘resources’ (such as a ‘person’ or a ‘prescription’) that can be re-used in many different ways – a message, an on-line service or a document. It is based on the RIM, but also borrows concepts from other models such as openEHR archetypes. It also natively supports JSON, which is the structure of choice for mobile and many web-based solutions. It has been designed from the beginning to be able to meet the needs of existing v2 and CDA users.

I will expand on these points at the CHIMA conference in June with two talks:

· One that discusses the use and future of ‘messaging’ standards – v3, v3, CDA and FHIR.

· The other will have a wider focus on standards that are useful in the EHR context – including standards from other organizations.
